The solution of Lidocaine 2% with Adrenaline 1:50.000, is the one that shows the greatest potency in the duration of pulp anesthesia. This is achieved by the high concentration of Adrenaline it contains. According to ADA(20), the maximum recommended dose is 5.5 carpules of 1.8 c.c. This solution is very useful for:
Achieving infiltrative anesthesia with hemostasis, due to the high concentration of Adrenaline and thus achieving a better view of the operative field and reducing the time of surgeries. To reinforce a periapical infiltration, to achieve pulp anesthesia, if previously infiltrated with a less potent lidocaine anesthesia and this has failed. In this way we increase the solution at the point of anesthesia but also increase the potency due to the greater adrenaline in the solution, and we do not mix two different anesthetics.
